Qualifications

• Performs the Nursing Process (assessment; diagnosis; outcomes identification; planning; implementation and evaluation) using evidence-based practice and critical thinking to individualize care.
• Delivers relationship-based patient-centered care that respects population characteristics (age, gender, disease) and cultural diversity.
• Partners with the patient and significant others via appropriate methods (bedside shift report; interdisciplinary rounds; just in time and planned patient teaching).
• Practices using current clinical practice standards.

Responsibilities

• Coordinates delivery and documentation of safe, high-quality patient care through interdisciplinary communication and timely documentation.
• Demonstrates teamwork and collaboration by practicing in a fiscally responsible manner and supporting recruitment/retention of healthcare team members.
• Participates in performance improvement initiatives and supports shared governance at multiple levels.
• Serves as a preceptor/mentor for other professionals to ensure a qualified workforce.
