• Formulates a plan of care based on trends of similar patients, including education and discharge planning.
• Tailors and prioritizes caring to individual needs, including cultural/ ethical/ spiritual needs.
• Participates in planning routine transitional health care needs.
• Responds to changes in patient needs with holistic care using clinical judgment.
• Adapts teaching to individual patients and families; collaborates to identify outcomes.
• Ensures efficiency in care delivery; sets priorities and resolves problems.
• Advocates for patient rights and resolves conflicts.
• Identifies variances from expected outcomes and revises care plan.
• Documents all patient care activities per standards; supports the care team.
• Communication and Collaboration: communicates with team, patients, and families to maximize outcomes.
• Delegates care activities and assists with staff assignments for productivity.
• Keeps updated on unit initiatives and applies team outcomes to practice.
