Responsibilities

• Collaborates with CareTeam to provide skilled nursing care and ensure patient needs are met.
• Assesses patient needs, plans care, and coordinates assignments with the team.
• Administers medications according to policy and documents outcomes accurately.
• Participates in discharge planning and patient/family education.
