Job summary

ZipStaff Inc. is seeking an experienced Primary Care/PACT RN for a local contract at the Miami VA Medical Center in Miami, FL. The role delivers comprehensive outpatient nursing care, patient education, and care coordination to Veterans in a multidisciplinary setting.

The RN will work with physicians and other professionals to ensure timely, coordinated, patient-centered care, with emphasis on chronic disease management and preventive health in a Veteran-focused environment.

Qualifications

• Current RN license in any U.S. state.
• AHA BLS certification.
• Minimum four years of current nursing experience, including care coordination (2+ years inpatient and 2+ years outpatient).
• Experience managing chronic conditions and coordinating care across teams.

Responsibilities

• Conduct comprehensive nursing assessments and evaluate patient needs.
• Coordinate patient care across primary care and specialty services.
• Support management of chronic conditions (diabetes, hypertension, CVD).
• Provide patient education on medications, treatment plans, and preventive care.
• Assist with outpatient procedures and clinical interventions.
• Maintain accurate EMR documentation.
