The U.S. Department of Veterans Affairs seeks a Complex Patient Aligned Care Team (PACT) Physician to be based at the Daytona Beach VA Clinic in Daytona Beach, FL. The role involves managing a panel of patients with complex chronic conditions and delivering gender-specific primary care.

Responsibilities include coordinating care within a PACT, reducing ED visits, ensuring timely documentation, and maintaining licensure. Travel to other facilities may be required as needed.

Responsabilidades

• Provide comprehensive gender-specific care under the PACT model with intensive case management.
• Manage chronic conditions and develop treatment plans according to accepted guidelines.
• Collaborate with a Patient Aligned Care Team (PACT) to deliver accessible, coordinated, continuous care.
• Work to reduce recurrent ED visits and hospital readmissions through coordinated care planning.
• Ensure timely documentation and complete administrative work in accordance with facility policy.
• Prescribe medications according to the approved formulary and in line with federal and state law.
• Participate in CME and maintain licensure; support peer review via chart reviews.
• Attend primary care and medical staff meetings and engage in inter-disciplinary collaboration.
• Be flexible with work schedules and potential reassignment to other services/locations as needed.
• Travel may be required for this position.
