Job summary

Aptive Resources, in partnership with Valor Healthcare, is hiring a Clinical Associate to provide direct clinical support within the PACT model at the proposed Yreka, CA CBOC, pending VA contract award. This is a Full-Time Contract position, on-site at 807 S.

Main St., Yreka, CA 96097, with a Monday–Friday schedule and clinic operating hours. Qualifications include LPN/LVN with active license and BLS, or VA Health Technician/Nursing Assistant credentials, plus credentialing and background checks

Qualifications

• LPN/LVN license active and unrestricted; BLS current.
• Ambulatory care experience preferred.
• VA Qualifications for Nursing Assistant or VA Health Aid/0640 series accepted.

Responsibilities

• Provide evaluation and care consistent with licensure/certification.
• Collaborate with PACT staff on care plans.
• Manage clinic workflow and room placements.
• Perform vitals and assist procedures.
• Maintain patient confidentiality and infection control.
• Serve as backup for telehealth as trained.
• Complete tasks without delaying patient care.

Job Summary

Clinical Associate – LPN/LVN or Health Technician/Nursing Assistant

Overview: Aptive Resources, in partnership with Valor Healthcare, is hiring a Clinical Associate to provide direct clinical support within the PACT model at the proposed Yreka, CA CBOC, pending VA contract award.

Location:On-Site | Yreka VA Community-Based Outpatient Clinic (CBOC), 807 S. Main St., Yreka, CA 96097

Schedule:Monday–Friday, 7:30am–5:30pm (Clinic Operating Hours); 8:00am–4:30pm Patient Schedule Hours

Employment Type:Full-Time Contract

Primary Responsibilities

• Provide evaluation and care consistent with licensure/certification to patients assigned to PACTs
• Collaborate with PACT staff on comprehensive health care and care management plans
• Manage clinic workflow and ensure timely placement of patients in examination rooms
• Perform vitals, point-of-care testing, and assist with procedures
• Demonstrate knowledge of medical terminology, aseptic technique, infection control, and patient confidentiality
• Receive training to serve as backup for telehealth services (e.g., Store & Forward, Clinical Video Telehealth)
• Complete assigned tasks within the clinic session without delaying patient care

Minimum Qualifications

• LPN/LVN: Current, full, active, unrestricted license; current BLS certification; ambulatory care experience
• OR Health Technician/Nursing Assistant: Meets VA Qualification Standards for Nursing Assistant, or OPM Health Aid and Technician Series 0640
• Must hold a current, full, active, and unrestricted license/certification (as applicable to the position) in California, or otherwise meet VA Qualification Standards for the position.
• Must successfully complete VA credentialing/privileging review and a background investigation (cleared through OPM via the VA Security and Investigations Center) prior to starting work; PIV credentialing required.
• Must complete required VA mandatory training and maintain current licensure/certification documentation annually.
