Responsibilities

• Coordinate with the patient care team to review clinic utilization using reports and maintain clinic readiness.
• Develop and maintain effective communication with patients, interdisciplinary teams, VA facilities, and external agencies; facilitate secure messaging and follow-up tracking.
• Assist with patient discharge communications between inpatient and outpatient settings and coordinate consults and tests as needed.
