UnityPoint Health’s PACE Clinic is seeking a Driver to provide safe transportation for elderly participants to appointments and program activities. You will assist with mobility needs, monitor wellbeing, and follow schedules and safety procedures while collaborating with the care team.

Under supervision, you will maintain vehicles and participate in on-call rotations to support reliable transport for frail participants within the PACE program.

Competenze

• High School diploma or GED required.
• 1 year of experience working with frail/elderly population preferred.
• Must possess and maintain current Basic Life Support (BCLS) within 30 days of hire.

Mansioni

• Provide safe, reliable transportation for PACE participants to appointments and program activities while delivering exceptional customer service.
• Assist participants with mobility needs, including wheelchair transport, ambulation, transfers, and home access.
• Monitor participant well-being, communicate changes in health or home conditions, respond appropriately to emergencies, and help create a welcoming, supportive environment.
• Follow transportation schedules, safety procedures, and all traffic laws while working independently and collaboratively with the care team.
• Maintain vehicles in a safe, clean, and ready-to-operate condition through daily inspections, fueling, and routine safety checks.
• Monitor vehicle performance, report maintenance needs, and support reliable transportation services across the PACE program.
• Participate in an after-hours on-call rotation as needed.
