Job summary

Bluegrass Care Navigators is seeking a PACE Social Worker to provide and coordinate psychosocial services for participants and their families across diverse settings, including the PACE Center, home, hospital, and ALF. The role emphasizes holistic care and strong collaboration with an interdisciplinary team.

You will deliver community-based case management and work with a network of local resources to support participants’ well-being and independence.

Qualifications

• MSW degree; licensed in Kentucky (CSW or LCSW).
• Minimum two years in social services in a case management role.
• At least one year of experience with frail and elderly populations.
• Proven ability to network with community resources.
• Strong communication, organizational, and interpersonal skills.
• Ability to manage multiple priorities with discretion.

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Position Summary

The PACE Social Worker provides and coordinates psychosocial services for participants and their families in the PACE Center, home, hospital, nursing home, or assisted living facility (ALF). The role also provides community-based case management services for program participants.

Position Qualifications

• Is a graduate of a master’s program in Social Work accredited by the Council of Social Work Education; and is licensed by the state of Kentucky as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).
• A minimum of two years’ social service experience working in a case management capacity.
• Must have at least one year of experience working with the frail and elderly.
• Strong demonstrated capabilities in networking with community resources, and strong knowledge of such resources.
• Strong communication skills, including face-to-face, telephone, and computer skills.
• Strength in attention to detail and organization.
• Must be able to interact successfully with participants, families, and other agencies to present a positive and professional image.
• Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team.
• Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
• Ability to work unsupervised exercising a high degree of discretion & independent judgment.
• Ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.).
