myPlace Health in Downtown Los Angeles is seeking a Scheduler for its PACE program. The role coordinates participant appointments across medical, dental, therapy, and specialty services, ensuring alignment with individualized care plans and regulatory timelines.

You will manage referrals, authorizations, and transportation, while coordinating with the interdisciplinary care team to ensure smooth onboarding and ongoing care.

Formación

• Education: A High School Diploma or GED is required.
• Experience: At least 2 years in medical scheduling, healthcare administration, or a related field.
• Language: Bilingual in English and Spanish to serve a diverse community.
• Technical: Experience using Electronic Medical Records (EMR) to keep schedules accurate.

Responsabilidades

• Coordinate and Schedule Participant Appointments – Manage medical, dental, therapy, and specialty appointments with alignment to care plans and regulatory timeframes (PACE, CMS, DHCS).
• Facilitate External Appointments – Schedule visits with specialists, diagnostic centers, and hospitals, ensuring referrals, authorizations, and transportation are in place; track follow-ups and inform caregivers and the IDT.
• Organize New Participant Orientation & Initial Assessments – Schedule orientation and ensure assessments across disciplines; identify transportation needs and coordinate with Transportation Team.
• Optimize Scheduling Across Care Settings – Act as the quarterback of scheduling, minimize conflicts, and optimize schedules for efficiency.
• Collaborate with Key Teams – Work with Transportation, Home Care, Medical Records, and Clinic teams to remove scheduling barriers.
• Liaison Between Key Departments – Serve as primary contact between Senior Center Manager, Clinic Staff, Medical Records, Home Care, and Transportation Coordinators.
• Participant Communication & Follow-Up – Place confirmation calls for external provider appointments with necessary details and support.
• Manage Scheduling Conflicts & Urgent Requests – Identify and resolve conflicts, cancellations, and urgent scheduling needs to ensure continuity of care.
• Maintain Accurate Documentation – Enter and update appointment details in EMR or scheduling software in compliance with standards.
• Other duties as assigned – Adapt to evolving needs of the PACE program.
