myPlace Health is seeking a Scheduler for our Downtown Los Angeles PACE program. This temporary full-time onsite role coordinates participant appointments, medical and therapy services, and transportation, ensuring care plans align with regulatory timeframes.

You will support an interdisciplinary team, connect with families, and keep schedules precise in our EMR. The ideal candidate has 2+ years in medical scheduling, is bilingual in English and Spanish, and can manage multiple tasks with warmth

Competenze

• Education: A High School Diploma or GED is required.
• Experience: At least 2 years in medical scheduling, healthcare administration, or a related field.
• Language: Bilingual in English and Spanish to serve a diverse community.

Mansioni

• Coordinate and Schedule Participant Appointments – Manage and confirm medical, dental, therapy, and specialty appointments while ensuring alignment with individualized care plans and regulatory timeframes (PACE, CMS, DHCS).
• Facilitate External Appointments – Schedule participant visits with specialists, diagnostic centers, and hospitals, ensuring all necessary referrals, authorizations, and transportation arrangements are in place.
• Organize New Participant Orientation & Initial Assessments – Schedule orientation day for new participants, ensuring assessments across multiple disciplines.
• Optimize Scheduling Across Care Settings – Act as the coordinating lead to minimize conflicts and optimize both participant and staff schedules.
• Collaborate with Key Teams – Work with Transportation, Home Care, Medical Records, and Clinic teams to remove scheduling barriers.
• Liaison Between Key Departments – Serve as the primary contact between Senior Center Manager, Clinic Staff, Medical Records, Home Care, and Transportation Coordinators.
• Participant Communication & Follow-Up – Confirm appointments with participants and provide necessary details and support.
• Manage Scheduling Conflicts & Urgent Requests – Resolve conflicts, last-minute cancellations, and urgent scheduling needs.
• Maintain Accurate Documentation – Enter and update appointment details in the EMR or scheduling software.
