Job summary

Valir Health is seeking an Occupational Therapist to evaluate and treat seniors within the PACE program in Oklahoma City, focusing on independence and meaningful daily activities.

The OT will develop individualized plans, promote functional improvement, and support participants to maintain independence in the PACE care model. Requires OT degree and an active license; NBCOT preferred and CPR required.

Qualifications

• Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program required.
• Current unrestricted Occupational Therapist license in the state of practice required.
• NBCOT certification preferred.
• Current CPR certification required.
• Minimum one (1) year of clinical experience preferred; geriatric, rehabilitative, or PACE experience strongly preferred.

Responsibilities

• Evaluates participants’ cognitive, physical, psychosocial, and perceptual functioning to promote independence and maximize participation in meaningful daily activities.
• Develops and implements individualized treatment plans designed to enhance functional performance, improve quality of life, and support participants in maintaining the highest level of independence within the PACE model of care.
• Supports participants in maintaining the highest level of independence within the PACE model of care.
