Job summary

Valir PACE LLC is seeking a Nurse Practitioner or Physician Assistant to deliver comprehensive primary care within an interdisciplinary team. You will assess, diagnose, and manage acute and chronic conditions while helping participants stay safely in their homes.

The role emphasizes collaborative care planning, preventive health initiatives, and transitions of care in a CMS-compliant program. You will work with physicians, nurses, therapists, social workers, pharmacists, and other clinicians to

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Current Oklahoma NP or PA license in good standing.
  • Current DEA registration.
  • Current BLS certification.
  • Eligible for Medicare and Medicaid credentialing.
  • Valid driver’s license with reliable transportation for community visits.

Responsibilities

Working collaboratively with the Medical Director and Interdisciplinary Team (IDT), this role assesses, diagnoses, treats, and manages acute and chronic medical conditions while supporting participants' goals of remaining safely in their homes and communities whenever possible.The APP participates in care planning, preventive health initiatives, transitions of care, and ongoing management of medically complex older adults in accordance with CMS regulations, state practice requirements, and organizational policies.QualificationsRequiredGraduate of an accredited Nurse Practitioner or Physician Assistant program.Current Oklahoma NP or PA license in good standing.Current DEA registration (or ability to obtain).Current BLS certification.Eligible for Medicare and Medicaid credentialing.Valid driver's license with reliable transportation for community visits.PreferredOne (1) year of experience working with frail or elderly adults, or completion of approved training in the care of frail older adults as required under PACE regulations.Primary Care, Geriatrics, Internal Medicine, Family Practice, or related clinical experience.Experience working within an interdisciplinary healthcare team.
