Job summary

Central Valley PACE is seeking an Intake Specialist to oversee enrollment for new participants at the Modesto center. The role involves educating potential participants and caregivers about PACE benefits and coordinating assessments.

You will gather medical and social information, conduct eligibility screening, and guide enrollment through the LOC process, coordinating with caregivers and the interdisciplinary team to ensure a smooth intake experience.

Qualifications

• Valid California Driver’s License, acceptable driving record and vehicle insurance.
• Ability to discuss and explain patients’ rights prior to enrollment into PACE.
• Must complete the State of California DHCS PACE and Medi-Cal Marketing Certification exam within 60 days of hire.
• Strong communication and presentation skills in English and Spanish.
• Marketing/Sales experience is preferred.
• Proficient with Microsoft Office suite.

Responsibilities

• Provides courteous and friendly customer service in person and over the phone, educating callers about available services.
• Conducts program eligibility screening, phone intake, education and enrollment assistance to determine needs and eligibility.
• Responsible for documenting medical and social indications of new participants to establish care needs at enrollment.
• Oversees enrollment of participants through the level of care (LOC) process.
• Educates potential participants and their caregivers on the benefits of Central Valley PACE.
• Tours participants and family caregivers at the PACE centers and explains services.
• Presents enrolled cases to the interdisciplinary team at centers.
• Develops relationships to secure face-to-face meetings with enrollment specialists.
• Interacts professionally with geriatric individuals including those with cognitive decline or frailty.
• Identifies targeted areas to invite potential participants and conducts mail campaigns.
• Collects patient demographic information and required paperwork throughout enrollment.
• Observes changes in participants and reports to the licensed nurse.
• Other projects and duties as assigned.
