Job summary

Golden Valley Health Centers seeks a licensed RN to conduct level of care assessments, provide clinic nursing services, and participate in the interdisciplinary care team in California’s Central Valley.

Initially based and trained at the Modesto center with Stockton as the eventual location; schedule Monday–Friday 8:00am–5:00pm. Compensation ranges from $49.92 to $57.79 per hour with comprehensive benefits.

Qualifications

• Excellent written and verbal communication skills.
• Ability to lead and supervise effectively.
• Data collection skills and knowledge of basic statistical principles.
• Proficient in Microsoft Excel.

Responsibilities

• Serve as liaison for DHCS LOC assessments.
• Conduct full medical and environmental in-home assessments.
• Prepare and oversee semi-annual level of care on-site audits.
• Develop a plan of care with the interdisciplinary team.
• Educate participants and caregivers on disease processes and self-care.
