Job summary

Elderhaus PACE in North Carolina is seeking a Nurse Practitioner or Physician Assistant to provide primary care to older adults enrolled in the program. You will perform comprehensive assessments, manage medical care, and coordinate with an interdisciplinary team to support independence and quality of life.

Qualifications

• Current and unrestricted North Carolina RN or PA license
• Certification as a Nurse Practitioner or Physician Assistant
• Current CPR certification
• Valid driver''s license and reliable transportation
• At least 1 year of experience with frail older adults
• Strong clinical assessment, communication, critical thinking, and care coordination skills

Responsibilities

• Conduct comprehensive health assessments for new participants
• Perform scheduled reassessments and monitor health status
• Develop, implement, and monitor individualized treatment plans
• Evaluate and treat acute and chronic conditions
• Order, interpret, and follow up on diagnostic tests
• Prescribe and manage medications
• Coordinate care with specialists and facilities
• Educate participants and caregivers on disease management
• Maintain clinical documentation and coding
• Participate in care planning and quality initiatives
• Support staff education and maintain high clinical standards
