CBH Care in Emerson, New Jersey, is seeking a full-time healthcare support professional to provide supervision and case management for assigned consumers. The role includes helping with daily living skills, budgeting, and linking to benefits, plus medication education and monitoring self-administration.

The candidate will contribute to treatment plans and document information in electronic records while adhering to the agency’s driving policy for safe transportation.

Qualifications

• BSW in social work required.
• BA/BS in psychology, criminal justice, sociology, or related field required.
• BA in an unrelated field with two years of previous experience acceptable.
• AA in a related field with two years of previous experience acceptable.
• Previous experience is desirable.
• Valid New Jersey driver’s license required.

Responsibilities

• Provides continuous supervision and case management services to assigned consumers on caseload.
• Assists consumers with activities of daily living skills, budgeting, and linkage to applicable benefits.
• Provides consumers with medication education and observes their self-administration of medication.
• Adheres to agency driving policy when transporting consumers in a safe and responsible manner.
• Provides input to treatment and service plans and assists with their implementation.
• Documents all relevant information to the consumers' electronic chart records.
