Job summary

PharMerica is seeking an Overnight Pharmacist (PRN) in Raleigh, NC to ensure safe and effective medication use in long-term care settings. The role involves reviewing orders, coordinating with healthcare teams, and maintaining accurate resident records.

The ideal candidate has a PharmD preferred background with a Bachelor's in Pharmacy, at least one year in long-term care, and strong attention to detail and communication.

Qualifications

• Bachelor's Degree in Pharmacy required; PharmD preferred.
• One year experience in long-term care.
• Licenses/certifications as required by regulations.
• Detail oriented; goal oriented; team player; good communication.

Responsibilities

• Evaluates physician medication orders for drug appropriateness, dosage, interactions and administration route.
• Maintains patient/resident database with medical history and concurrent medications.
• Ensures monitoring laboratory tests for high-risk conditions are ordered and reviewed.
• Collaborates with healthcare team to provide clinical and technical support.
• Documents communications with physicians, nurses, and patients in the medical record.
• Implements policies for safe procurement, storage, distribution and disposal of drugs.
• Obtains and maintains required documentation per regulatory agencies.
• Informs Pharmacy Manager of medication-related errors.
• Assists staff with safe procurement, storage, administration and disposal of drugs.
• Performs other tasks as assigned.
