Qualifications

• Bachelor’s degree in Pharmacy is required; PharmD preferred.
• One year of experience in long-term care.
• Licenses/Certifications as required by government regulations and business needs.

Responsibilities

• Evaluates physician medication orders for drug appropriateness, dosage, interactions, and route.
• Maintains patient/resident medical histories and current medications.
• Ensures appropriate laboratory tests for high-risk conditions are ordered and reviewed with the care team.
• Collaborates with the healthcare team to provide clinical and technical support.
• Documents communications with physicians, nurses, and patients in medical records.
• Implements policies for safe procurement, storage, distribution, and disposal of drugs.
