Responsibilities

• Initiates the plan of care and requests revisions as needs change.
• Incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Intervenes for changes in patient condition and reports to physician or supervisor.
• Administers medications and documents outcomes.
• Reviews plan of care and coordinates education, care, and discharge planning.
