Job summary

The role entails comprehensive patient assessments, medical management, and coordination with a multidisciplinary team to improve outcomes for seniors in their own homes, with flexibility in scheduling and a focus on compassionate care.

Qualifications

• Must hold NP licensure and NY state registration.
• Excellent communication and follow-up skills.
• Proficient with Microsoft Excel, data entry, and reporting.
• Experience with geriatric patients and home care preferred.
• Knowledge of medical documentation practices.

Responsibilities

• Examine patients and order appropriate diagnostic workups.
• Assess members with acute change in condition promptly.
• Maintain complete and accurate medical records.
• Consult with interdisciplinary team to create care plans.
• Educate patients on disease processes, medications, and adherence.
• Provide medical management of home-based patients.
