PharMerica in Horsham, PA is seeking a full-time Pharmacist for a closed-door, non-retail setting serving long-term care and senior living clients. You will evaluate orders, provide clinical recommendations, and maintain resident profiles.

This role features an overnight 7 on/7 off schedule with a $6/hour shift differential, competitive benefits, tuition assistance, and career growth opportunities within a growing company.

Qualifications

• Bachelor’s degree in Pharmacy; Pharm D preferred.
• Active pharmacist license in the state of practice.
• Minimum 1 year of long-term care experience.
• Knowledge of CMS, DEA, FDA guidelines.
• Proficiency with EMR and pharmacy information systems.
• Strong clinical judgment and communication skills.

Responsabilités

• Evaluate medication orders for drug safety, dosing, interactions, and route of administration.
• Provide clinical recommendations to physicians and facility staff on therapy changes.
• Maintain patient/resident profiles including medical history, labs, and current meds.
• Ensure monitoring labs for high‑risk meds are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and others to provide clinical support.
• Document communications and interventions in the medical record.
• Enforce policies for safe procurement, storage, administration, and disposal of meds.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors to Pharmacy Manager as required.
• Educate facility staff on safe medication handling.
