Job summary

BAYADA Home Health Care in New Castle County, Delaware is seeking an LPN for our Transition Team to help infants, children, and adults receive complex care at home. This overnight, full-time or part-time role supports care transition from hospital settings to ongoing home care, delivering quality care and training to the BAYADA team.

Requirements include a current LPN license, NICU/PICU/ICU or home care trach/vent experience, CPR certification, graduation from an approved nursing program, and

Qualifications

• Current LPN license valid in the state.
• Experience in NICU/PICU/ICU or home care trach/vent required.
• CPR certification current.
• Graduate from approved/accredited nursing program.
• Ability to travel throughout New Castle County.
• Available for full-time or part-time with overnight shifts.
• Willingness to work in a teaching setting.

Responsibilities

• Deliver quality home care to pediatric and adult clients.
• Provide extensive training to BAYADA care team.
• Assist transition of care from hospital to home care.
• Work night shifts overnight.
• Collaborate with office team.
