Qualifications

Responsibilities

• Completes assessments of patient and family to determine skilled home care needs.
• Uses health assessment data to determine nursing diagnoses.
• Directs conversation and guidance for self-care and referrals as needed.
• Provides direct/triage client/patient care per State Nurse Practice Act.
• Maintains documentation and coordinates with physicians and care team.
