Job summary

Beth Israel Lahey Health seeks a social worker to join the overnight SW primary response team. The role provides psychosocial screening, assessment, counseling, and discharge planning support for patients and families in a fast-paced hospital setting.

You will collaborate with nurses, physicians, case managers, and social work colleagues, document in the medical record, participate in CQI efforts, and may supervise interns or staff under LICSW supervision.

Qualifications

• Master's degree in Social Work.
• Experience with computer systems and MS Office applications (Outlook, Word, Excel, PowerPoint, OneDrive, SharePoint).
• Licensed at an LCSW or higher.

Responsibilities

• Screens and triages patients who request service, are referred, or meet screening criteria.
• Provides clinical services to address psychosocial issues, maximizing well-being and appropriate healthcare use.
• Collaborates with the multidisciplinary team to facilitate discharge planning and outpatient continuity of care.
• Provides counseling and psychotherapeutic services and documents in the medical record.
• Participates in CQI efforts and may supervise staff or interns when licensed.
