Qualifications

Responsibilities

• Support individuals based on their service plan, assisting with hygiene, bathing, toileting, and mealtime needs.
• Teach self-control and self-management per training.
• Complete client logs and charts within shift or 24 hours with supervisor approval.
• Help individuals develop a daily schedule of activities tailored to their needs.
• Arrange appointments and transportation with proper documentation.
• Promote safety and follow all safety procedures with clients and team.
