Job summary

Advocate Health seeks a Pharmacist to provide safe, effective pharmaceutical care in their practice area, aligned with region-specific career ladder expectations.

You will oversee medication therapy, educate patients, ensure safety, review orders, and collaborate with the medical team to optimize formulary use.

Requires Georgia licensure or reciprocity and a PharmD or B.S. in Pharmacy; PGY1 preferred. Full-time overnight, 35 hours/week; benefits eligible; sign-on bonus may apply.

Qualifications

• Licensed in Georgia or eligible for reciprocity required.
• Bachelor’s or Doctorate in Pharmacy from ACPE-accredited school or FPGEC NABP certificate.

Responsibilities

• Oversees pharmaceutical care including evaluating literature and designing medication regimens.
• Educates patients and caregivers about medications, adverse effects, compliance, and administration.
• Designs and monitors patient-specific pharmacotherapeutic regimens and implements recommendations.
• Reviews medication orders and prescriptions for accuracy and safety.
• Supports medication safety and quality improvement activities within the department.
• Collaborates with the healthcare team on formulary adherence and medication-use processes.
• Prepares and oversees storage and handling of medications in compliance with regulations.
• Serves as a preceptor and mentors pharmacy teammates, residents, and students.
