Pathway Society, Inc. in San Jose, CA, is seeking an Overnight Care Coordinator to supervise clients at Pathway House during the overnight shift.

You will monitor the residential milieu, ensure safety, assist clients, document actions and needs, perform urinalysis as needed, and coordinate with counselors to support treatment goals while upholding professional boundaries and HIPAA compliance. This role requires physical ability to respond to client needs, climb stairs, and pass a criminal

Qualifications

• One-year practical experience in drug/alcohol or mental health field preferred.
• Basic understanding of treatment and rehabilitation techniques.
• Excellent written and verbal communication skills.
• Ability to work effectively within a team as well as alone.
• Must pass a criminal records clearance as required by Santa Clara County.

Responsibilities

• Monitor the residential milieu overnight by conducting rounds in timeframes prescribed by agency policy and procedure.
• Ensure the safety and security of clients and the facility.
• Assist clients with their needs and refer them to their counselor for clinical support.
• Maintain continuity among work teams by documenting and communicating actions, irregularities and ongoing client needs in chart and shift log notes.
• Perform urinalysis testing as needed.
• Educate and remind clients of the rules, policies and procedures of the facility.
• Establish appropriate boundaries amongst clients and staff, ensuring professional and courteous interactions and fostering opportunities to implement newly developed life skills.
• Assist with medication monitoring, counting, and compliance as needed.
• Set up, serve, and monitor client breakfast.
• Wash linens and perform light household tasks.
• Ensure cleanliness of office and client treatment areas.
• Perform additional duties as assigned by the program director

Job Location: Pathway House - San Jose, CA 95112Salary Range: $23.00 - $27.00 HourlyPosition Description: The overnight care coordinator provides supervision and support to clients in the residential treatment facility during the overnight shift.

Qualifications

• One-year practical experience in drug/alcohol or mental health field preferred (employment, internship or volunteer work); preferred RADT, working towards certification in drug and alcohol counseling
• Basic understanding of treatment and rehabilitation techniques
• Excellent written and verbal communication skills and demonstrated ability to work effectively within a team as well as alone
• Ability to work effectively with people differing in value systems, ethnicity, cultural backgrounds, language capabilities and disabilities
• Exercise good judgment and respond calmly in emergency situations
• Ability to work effectively; independently and within a group, with limited supervision
• Must maintain the highest standards of professional ethics and behavior as described in the Pathway Society handbook.
• Must be able to physically respond to clients’ needs, climb stairs and walk through the facility throughout the shift
• Must pass a criminal records clearance as required the County of Santa Clara Department of Alcohol and Drugs regulations.
• Receive training and pass certification testing in the areas of HIPAA, 42 CFR and CPR/First Aid once county clearance is received.

Physical Requirements

• Climbing; ascending or descending stairs, ramps, using feet and legs and/or hands and arms.
• Kneeling; bending legs at knee to come to a rest on knee or knees.
• Crouching; bending the body downward and forward by bending leg and spine.
• Reaching; extending hand (s) and arms above head in any direction.
• Hearing; perceiving the nature of sounds at normal speaking levels with or without correction, ability to receive detailed information through oral communication, and to make the discriminations in sound.
• Lifting; able to lift, carry, push, pull or otherwise move objects up to 20 pounds.
• Visual acuity to perform an activity such as: preparing and analyzing data and figures and viewing a computer terminal.
