Job summary

SSTAR is seeking an Outreach Worker for Project Aware to conduct outreach, provide secondary services, and assist providers in meeting clients’ needs on the medical unit and in the community. The role includes operating a van or RV safely between outreach locations and requires reliable transportation and a valid driver’s license.

Shift: Monday - Thursday 12:00pm - 4:00pm and Friday 8am - 4pm. Benefits include health and disability insurance, paid time off, and opportunities for CEUs.

Qualifications

  • HS Diploma or GED required.
  • Two years of full-time experience in health care, medical care or public health education in a recognized hospital, medical facility, or agency.

Responsibilities

  • Develop and sustain positive relationships with residents, community leaders, local groups and organizations.
  • Interact with clients to determine knowledge about harm reduction and attitudes toward HIV/HCV/STIs.
  • Provide overdose education and distribute naloxone to clients and the community.
  • Link clients to medical care and appropriate treatment; initiate referrals.
  • Enter program activity data accurately and timely.
  • Attend trainings and regional meetings; stay informed on trends and research.

Education

The Outreach WorkerIn Project Aware conductsoutreach among vulnerable populations, provides targeted secondaryservices and assists Provider to meet clients’ needs both on themedical unit and in the community. This role also includesresponsibility for safely operating and transporting the outreach vanor RV to and from designated outreach locations.

Must have reliable transportation and a valid driver's license.

Duties & Responsibilities

  • Developing and sustaining positive relationships with a range ofpeople, including residents, community leaders, local groups andorganizations, and local authorities
  • Interact with clients to determine their level of knowledge aroundharm reduction and their attitudes/ behaviors about HIV/HCV/STIs
  • Provide overdose education and distribute naloxone to both clientsand the community
  • Assess potential barriers to care and work with clients to reducebarriers resulting from lack of transportation, mental illness,substance use, etc.
  • Link clients to medical care and appropriate treatment.
  • Initiate appropriate client referrals both internally andexternally to the organization.
  • Collect and accurately enter program activity data into theappropriate system in a timely manner
  • Attend relevant training courses and participate in all assignedregional meetings.
  • Stay informed about current trends, development, and research
  • Adheres to the work schedule. Obtains supervisor approval inadvance of schedule conflicts as well as late arrivals or early departures.
  • Records all hours worked by the last day of the pay period.
  • Responds appropriately to all Code Calls
  • Follows the incident reporting policy and procedure in reportingany unsafe/unusual safety issues.
  • Treats patients, and colleagues with dignity and respect; honorseach person’s right to privacy/ maintains very strict patient andcolleague confidentiality.
  • Timely completes all trainings, certifications, re-credentialingand licensing renewals requirements.

Maintains current awareness of SSTAR’s communications bychecking, reviewing and
responding to work emails at minimumon a weekly basis. Asks for assistance when needing help
orto fully understand the email message.

  • Performs other duties/special projects assigned.
Experience and Skills:

Education & Experience

  • HS Diploma/GED

Two years of full-time experience in the field of health care,medical care or public health education in a recognized hospital,medical facility, or agency.

Pay Rate Range: $18.00 - 21.00/ hour
