Job summary

Community Health Resources seeks an Outreach Therapist for the Women’s REACH program in Willimantic, CT. This full-time role provides assessment and treatment, crisis intervention, and discharge planning across office, home, community, and residential settings.

The candidate holds a Master’s Degree or higher in a related field, with LMSW/LPC-A/LMFT-A or equivalent licensure; experience with trauma and substance use is preferred. Flexible supervision and additional program trainings are available.

Qualifications

• Master’s Degree or higher in a related field is required.
• Experience with trauma, mental illness and/or substance use disorders preferred.
• Licensure/certification: LMSW, LPC-A, LMFT-A required; LCSW/LMFT/LPC or Licensed Psychologist preferred.

Responsibilities

• Provides assessment and treatment interventions consistent with the program model.
• Conducts clinical and risk assessments.
• Collaborates with individuals to create individualized, person-centered treatment plans.
• Demonstrates competency in age-related or specialty issues for each population served.
• Works with clients to develop appropriate discharge plans utilizing natural support.
• Maintains collaborative relationships with family members and other service providers.
• Provides crisis assessment and intervention to monitor clients in crisis.
• Transports clients using agency or personal vehicles as required.
• Participates in weekly supervision, consultations, and meetings.
