Job summary

Central Outreach Wellness Center is seeking an Outreach Registered Nurse to operate mainly at an all-men's club in Cleveland Heights, performing STI testing, distributing condoms and marketing materials, and maintaining connections with the main clinics.

The role requires RN licensure in Ohio, CPR certification, and proficiency with EMR systems, with a focus on harm reduction and patient-centered care in a fast-paced environment.

Qualifications

• Active OH RN license required.
• Current CPR certification mandatory.
• HIPAA compliance is essential.
• Experience with Electronic Medical Records systems.
• Strong communication with patients and staff.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process.
• Collect health histories and document findings.
• Counsel patients on treatment plans including HCV, HIV, STIs, MAT, Narcan training.
• Notify patients of results and coordinate follow-up treatment.
• Support clinicians with duties such as prescription refills.
• Educate patients on harm reduction techniques and safe practices.
• Read, interpret, and make treatment decisions based on patient information.
• Collaborate with clinicians to determine best treatment plans.
• Maintain patient connectivity between the clinic and outreach site.
