OSF HealthCare in Peoria, IL is expanding a mobile MAT clinic to provide street and shelter medicine and manage opioid use disorder with FDA-approved medications. The role collaborates with interdisciplinary teams to deliver primary care, behavioral health screening, and care coordination in community settings.

Qualificações

• Licensed to provide advanced practice nursing services in Illinois or licensed physician assistant services in Illinois.
• Licensed by DEA and Illinois to prescribe narcotics, if applicable.
• BLS/ACLS Certified.
• Eligible as a provider for Medicare, Medicaid, and other federal health programs.
• Liability coverage participation through OSF programs.

Overview

JOB SPECIFICS

OSF HealthCare is developing a mobile unit to expand outreach medicine and serve as an extension to our existing physical Medication Assisted Treatment (MAT) clinic.

Position Highlights:

• Provide comprehensive care through street medicine, shelter medicine, and mobile MAT services, extending access to patients in community-based settings.
• Evaluate, diagnose, and treat opioid use disorder using FDA-approved medications and evidence-based recovery practices.
• Deliver acute care and transitional primary care services to underserved and vulnerable populations.
• Conduct behavioral health screenings and assessments for co-occurring mental health and substance use conditions.
• Assess social determinants of health and identify recovery support needs to promote long-term patient stability.
• Collaborate with interdisciplinary teams to provide care navigation, case management, patient monitoring, and whole-person support services.
• Facilitate referrals and warm hand-offs to specialty care providers, community resources, ancillary services, and insurance enrollment assistance.
• Promote harm reduction, overdose prevention, patient education, recovery support, and care coordination to improve health outcomes and engagement in treatment.
• Monday - Friday, day shift with some evening requirements.

• Full-time, clinical base hourly pay range for this position: $55.20 - $63.65. This position is salaried.
  • Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement

Qualifications

REQUIRED QUALIFICATIONS

• Licensed to provide either advanced practice nursing services in the State of Illinois as issued by the Illinois Department of Financial and Professional Regulation in accordance with the Illinois Nurse Practice Act OR physician assistant services as issued by the Illinois Department of Financial and Professional Regulation in accordance with the Illinois Physician Assistant Practice Act of 1987.
• Licensed by the Federal Drug Enforcement Administration and the State of Illinois to prescribe narcotics, if applicable.
• BLS/ACLS Certified.
• Must be eligible as a provider for Medicare, Medicaid, and other federal health programs.
• Provider shall meet all qualifications to participate in OSF's programs for professional malpractice and other liability coverage.

PREFERRED QUALIFICATIONS

Demonstrates the knowledge and skills necessary to provide patient care aligned to the preferences, goals, and needs of the patients served. Emergency services or urgent care experience is preferred.
