Job summary

The Bridge is seeking an Outreach & Engagement Coordinator to lead a Manhattan-wide team connecting disconnected and homeless individuals with care. The role collaborates with hospitals, shelters, and social service organizations to evaluate referrals and enroll participants promptly.

Preferred candidates include those with experience serving homeless or precariously housed populations, strong communication skills, and bilingual Spanish abilities.

Qualifications

• Bachelor's Degree in a related field.
• Excellent communication skills, both verbal and written.
• Bilingual Spanish is highly desired.

Responsibilities

• Lead a team across Manhattan to engage disconnected and homeless individuals into care with referrals from hospitals, shelters, and social services.
• Evaluate referrals from partners to determine eligibility for services.
• Serve as primary liaison to ensure timely enrollment and address referrals.
• Participate in community outreach on streets and subways and support participants transitioning from street to home.
• Build connections with community providers to connect eligible participants to services.
• Submit referral tracking documents to Directors and CBC and maintain accurate records.
• Coordinate staff scheduling and fulfill Outreach & Engagement responsibilities with Directors.
