Qualifications

  • Bachelor’s Degree in a related field.
  • Work Schedule: Saturday to Wednesday, 9 AM - 5 PM
  • Preferred experience with homeless or precariously housed populations.
  • Experience working with individuals with a behavioral health diagnosis preferred, but not required.
  • Excellent communication skills, both verbal and written.
  • Bilingual Spanish is highly desired.

Responsibilities

  • Lead a Manhattan-wide team to engage disconnected and homeless individuals into care by working with referral sources such as hospitals, shelters, and social service organizations.
  • Receiving and evaluating all referrals to determine eligibility for services.
  • Primary liaison for all referrals to ensure enrollment and timely processing.
  • Coordinate street and subway outreach; support transitions from street to home; advocate for participants.
  • Build and maintain connections with community providers to connect participants to services.
  • Complete and submit referral tracking documents to Directors and CBC.
  • Document accurately and timely.
  • Collaborate with Directors to manage staffing and duties.

Scope of Position

The Outreach & Engagement Coordinator will lead a team across Manhattan, connecting disconnected and homeless individuals with care through collaboration with hospitals, shelters, and social service organizations. This role involves evaluating referrals, ensuring timely enrollment, and managing community outreach efforts, while supporting participants in strengthening daily living skills and advocating on their behalf. Essential Position Functions
  • Leading a team of staff throughout Manhattan to engage disconnected and homeless individuals into care by working with referral sources such as hospitals, primary care physicians, shelters, substance use treatment programs, mental health providers, SROs, and social service organizations, in addition to canvassing throughout Manhattan subways and streets.
  • Receiving and evaluating all referrals from community partners, the SOS HUB, and OMH to determine eligibility for services.
  • Serving as the primary liaison for all referrals to ensure potential participants are cleared for enrollment, and working with staff to ensure referrals are addressed in a timely manner.
  • Participating in community outreach on the streets and subways; coordinating participant needs before and after their transition from street to home; enhancing daily living skills; accompanying participants to appointments; and advocating on their behalf when faced with discrimination or healthcare inequities. Member choice, harm reduction, non-coercion, flexibility, and person-centered care are essential elements of the SOS program model and should be central to all services delivered.
  • Building and maintaining connections with community-based providers to ensure eligible participants form meaningful relationships and connect to all appropriate services.
  • Completing and submitting all referral tracking documents to the Directors and CBC.
  • Providing accurate, complete, and timely documentation.
  • Working closely with the Directors of two SOS teams to ensure staff scheduling, task completion, and fulfillment of all Outreach & Engagement staff responsibilities.
