The Bridge Inc. in New York, NY, seeks an Outreach & Engagement Coordinator to lead a Manhattan-based team connecting homeless individuals with care across hospitals, shelters, and social services.

You will evaluate referrals, enroll participants, and support community outreach while promoting user-centered care. Ideal candidates have a BA/BS and experience with homeless or behavioral health populations, strong communication, and Spanish language skills.

Qualifications

  • Bachelor’s Degree in a related field.
  • Work Schedule: Saturday to Wednesday, 9 AM - 5 PM
  • Preferred experience with homeless or precariously housed populations.
  • Experience working with individuals with a behavioral health diagnosis preferred, but not required.
  • Excellent communication skills, both verbal and written.
  • Bilingual Spanish is highly desired.

Responsibilities

  • Lead a Manhattan-wide team to engage disconnected and homeless individuals into care by working with referral sources such as hospitals, shelters, and social service organizations.
  • Receiving and evaluating all referrals to determine eligibility for services.
  • Primary liaison for all referrals to ensure enrollment and timely processing.
  • Coordinate street and subway outreach; support transitions from street to home; advocate for participants.
  • Build and maintain connections with community providers to connect participants to services.
  • Complete and submit referral tracking documents to Directors and CBC.
  • Document accurately and timely.
  • Collaborate with Directors to manage staffing and duties.

Education
