Responsibilities

  • Establishes and maintains relationships with local service providers and employers.
  • Connects veterans experiencing homelessness and at-risk veterans and families with housing and services.
  • Conducts the initial screening of potential clients at the site.
  • Facilitates the admission and discharge processes for programs at the site.
  • Identifies goals of potential clients to ensure suitability for programs at the site.
  • Tracks and maintains a database of clients served by the site’s outreach team.
  • Performs interventions and crisis resolution with clients.
  • Supports and upholds program principles; communicates with Case Managers and staff.
  • Participates in weekly meetings regarding program goals.
  • Ensures grant and program compliance by verifying eligibility through reviewing documentation.
  • Compiles intake and verification documentation and ensures file completeness.
  • Maintains occupancy rate for the program through outreach.
  • Facilitates room assignments and changes for existing clients across all programs.
  • Facilitates communication with Case Management and Veteran Services departments.
  • Provides updates to Program Manager, Operations Manager, and Executive Director.
  • Assists in training of new Outreach Specialist and other staff as appropriate.
  • Plans, implements, supports and troubleshoots staff training on safety, toxicology testing processes, and other areas as needed.
  • Performs other duties as assigned.

  • Provides regular updates to Program Manager, Operations Manager, and Executive Director regarding admissions, client developments, changes, discharges, etc.
  • Assists in training of new Outreach Specialist and other new staff as appropriate
  • Plans, implements, supports and troubleshoots staff training on issues of safety, toxicology testing processes, and other areas as needed
