The Outreach Care Specialist is responsible for ensuring that appropriate member treatment plans are followed on less complex cases and for proactively identifying ways to improve the health of our members and meet quality goals. Responsibilities include coordinating follow-up care plan needs for members by scheduling appointments or enrolling members in programs, assessing member compliance with medical treatment plans via telephone or through on-site visits, identifying barriers to plan compliance and coordinating resolutions, identifying opportunities that impact quality goals and recommending process improvements, recommending treatment plan modifications and determining need for additional services in conjunction with case management and provider, coordinating identification of and referral to local, state or federally funded programs, coaching members on ways to reduce health risks, preparing reports to document case and compliance updates, and establishing and maintaining relationships with agencies identified in appropriate contract. The candidate must reside in Eastern or Central time zone. This role is 100% virtual with exception of required in-person training sessions. Shift options are 8:00am-5:00pm EST or 8:00pm-5:00pm CST. Minimum requirements include a high school diploma or equivalent and a minimum of 1 year related experience or equivalent combination of education and experience. Preferred qualifications include certified nurse assistant or certified medical assistant, BS/BA degree in a related field, strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, analytical skills, and bilingual or multi-language skills may be required for some areas. Salary range for certain locations is $18.31/hr-$34.69/hr.
