You will perform comprehensive assessments, develop care plans, educate patients and families, and collaborate with providers to promote healing and safety.

Qualifications

• Bachelor’s degree required.
• Wound Care Certification required.
• Ostomy Certification required.
• BLS Certification required.
• Minimum of 3–5 years of related nursing experience required.
• Wound care experience preferred.
• Proficiency with Microsoft Office Suite.
• EPIC knowledge preferred.

Responsibilities

• Assess patients and obtain health history.
• Perform vital signs and wound/ostomy screenings.
• Conduct advanced assessments of wounds, ostomies, and fistulas.
• Triage patient calls and provide clinical guidance.
• Collaborate with care teams to develop individualized care plans.
• Assist with ordering tests and medications and obtaining authorizations.
• Manage patient flow and prepare for procedures.
• Perform specialized treatments including wound care and ostomy management.
• Educate patients and families on wound care and self-management.
• Maintain inventory of wound care supplies and medications.
• Monitor clinical safety equipment and logs.
• Mentor and train new clinical staff.
• Round and consult with wound care providers.
