Job summary

Med Source Consultants seeks an Advanced Practice RN specializing in wound, ostomy, and continence care for an inpatient hospital setting in Manchester, CT. The role focuses on wound assessment, advanced dressings, ostomy management, and prevention initiatives within a collaborative care team.

The candidate must hold an APRN license in the state and WOCN certification with 3–5 years of relevant experience, preferably in acute care. This position offers an employed salary with benefits.

Qualifications

• Must hold APRN licensure in the applicable state.
• WOCN certification required; 3–5 years of wound/ostomy care experience.
• Experience in acute care or hospital-based consultation preferred.

Responsibilities

• Perform comprehensive wound assessments and classify wound etiology.
• Develop and implement wound care plans with advanced dressings.
• Lead pressure injury prevention initiatives with risk assessment.
• Provide ostomy care including preoperative marking and postoperative management.
• Educate patients and caregivers on self-care pre-discharge.
• Collaborate with hospitalists, surgeons, and nursing staff on care plans.
• Participate in rounds and case reviews to ensure quality.
• Serve as a clinical resource on wound, ostomy, continence care.
• Contribute to staff education on wound care and ostomy management.
• Maintain competencies with Nursing Education and onboarding.
• Support patient and family health literacy initiatives.
