Southern New Hampshire Medical Center is seeking an RN Outpatient Wound and Ostomy Nurse to provide specialized care for acute and chronic wounds, ostomies, and fistulas in an outpatient setting. You will assess patients, coordinate care with the interdisciplinary team, and support education and clinical excellence to optimize outcomes.

Requirements include a Bachelor’s degree, Wound Care and Ostomy Certifications, BLS, and 3–5 years of nursing experience.

Formación

• Bachelor’s degree required.
• Wound Care Certification required.
• Ostomy Certification required.
• BLS Certification required.
• 3–5 years of related nursing experience required.
• Wound care experience preferred.

Responsabilidades

• Assess patients and obtain relevant health history.
• Perform vital signs and wound/ostomy screenings.
• Conduct advanced assessments of wounds, ostomies, and fistulas.
• Triage patient calls and provide clinical guidance.
• Collaborate with providers to develop and coordinate care plans.
• Assist with ordering tests and medications and obtaining authorizations.
• Manage patient flow and prepare for procedures.
• Perform specialized wound care treatments (e.g., VAC therapy, dressings).
• Provide patient and family education on wound care and self-management.
• Mentor and train new clinical staff.
• Round and consult with wound care providers as needed.
• Utilize Doppler to assess peripheral pulses.
• Respond to emergencies and codes.
