Southern New Hampshire Medical Center in Nashua is seeking an RN Outpatient Wound and Ostomy Nurse to provide specialized care for wounds, ostomies, and fistulas in an outpatient setting, collaborating with providers and the care team to optimize outcomes.

You will perform assessments, wound care procedures including VAC therapy, educate patients and families, coordinate tests and authorizations, mentor new staff, and help maintain clinical safety.

Qualifikationen

• Bachelor’s degree required.
• Wound Care Certification required.
• Ostomy Certification required.
• BLS Certification required.
• Minimum of 3–5 years of related nursing experience required.
• Wound care experience preferred.
• Proficiency with Microsoft Office Suite.
• EPIC knowledge preferred.

Aufgaben

• Assess patients and obtain health history.
• Perform vital signs and wound/ostomy screenings.
• Conduct advanced assessments of wounds, ostomies, and fistulas.
• Triage patient calls and provide clinical guidance.
• Collaborate with providers to develop care plans.
• Assist with ordering tests and medications and obtaining authorizations.
• Manage patient flow and prepare for procedures.
• Perform specialized treatments (wound wraps, VAC therapy, dressings, ostomy/fistula management).
• Provide patient/family education on wound care and self-management.
• Mentor and train new clinical staff.
• Utilize Doppler to assess peripheral pulses.
• Respond to emergencies and codes.
