Job summary

Southern New Hampshire Medical Center in Nashua, NH is seeking an RN Inpatient Wound and Ostomy Specialist. You will provide expert consultation and direct patient care for wound, ostomy, and continence needs within an interdisciplinary team, while advancing clinical practice through education, prevention initiatives, and care coordination.

The role requires a BSN, NH RN license, WOCN/ostomy certifications, and 3–5 years of related experience.

Qualifications

• BSN required and current NH RN license
• Certifications in wound/ostomy care required or strongly preferred
• CWOCN certification preferred
• Minimum 3–5 years of related nursing experience
• Inpatient wound care experience preferred
• Completion of a WOCN-accredited program or equivalent experience

Responsibilities

• Provide specialized wound, ostomy, and continence care through assessment, treatment planning, and evidence-based clinical interventions.
• Collaborate with physicians and nursing staff to develop, implement, and document individualized care plans, including discharge planning.
• Educate patients, families, caregivers, and staff on wound prevention, ostomy management, postoperative care, and best practices.
• Support quality initiatives through wound surveillance, protocol development, product evaluation, and accurate documentation.
