Carrollton Health & Rehabilitation in Carrollton, TX, is seeking a licensed Physical Therapist or Physical Therapist Assistant to serve as Outpatient Coordinator. You will build relationships with physicians, office managers, and referral coordinators, educate them on services, discuss patient care, and coordinate outpatient therapy scheduling.

• Licensed PT or PTA
• Experience in SNF or home health beneficial

Responsibilities

• Build and strengthen relationships with current and prospective referral sources through face-to-face meetings with physicians, office managers, referral coordinators, and community members
• Identify and market to local primary and specialty physicians to address outpatient therapy needs
• Assist Director of Rehab with coordinating community events and marketing campaigns
• Manage outpatient therapy schedule to maintain POC and coordinate with therapists for rescheduling
• Provide therapy services to long-term care, skilled in-patient, and outpatient populations
• Provide therapy services in patients' homes, group homes, and ALF/ILF
