Axis Health System is seeking a Therapist for outpatient care in Durango, CO. The role involves designing and implementing individual care plans for patients within the program team, following policies and evidence-based models.

The ideal candidate will have experience with adult or child/adolescent therapy, with a focus on mental health or substance use treatment. Licensure eligibility in Colorado is preferred for licensed professional track.

Competenze

• Bachelor's degree in one of the recognized mental health professions.
• Minimum of 2 years’ experience in mental health field.
• Colorado License Eligible / Licensed LPC, LMFT, LCSW or PsyD in Colorado.

Mansioni

• Design and implement individual care plans for program patients.
