Job summary

Axis Health System seeks a Therapist for the Delta-Outpatient location to design and implement individual care plans for program patients within a collaborative team. The ideal candidate offers experience in adult or child & adolescent therapy with evidence-based mental health or substance use treatment approaches.

The role requires a 4-year degree with at least two years in mental health; master’s degree and Colorado licensure are preferred.

Qualifications

• Bachelor's degree in one of the recognized mental health professions.
• Minimum of two years’ experience in mental health field.
• Preferred Master’s degree and Colorado licensure (LPC/LMFT/LCSW/PsyD).

Responsibilities

• Design and implement the individual care plans for program patients as part of the program team.
• Participate in the program team to ensure adherence to policies and directives.
