Job summary

Pathways Inc. in Grayson, KY is hiring Outpatient Therapists to deliver assessment, treatment planning, and individual/family therapy within a collaborative behavioral health team.

You'll maintain active caseloads, ensure timely documentation, and adhere to licensing standards while pursuing licensure where needed. We offer professional growth, supervision, and a comprehensive benefits package.

Qualifications

• Master's degree in psychology, counseling, social work, or nursing.
• KY licensure (associate or independent) required; supervised licensure acceptable.
• Active supervision requirements per licensing board.
• PMH accreditation (ANCC) preferred but not required.
• Behavioral health experience preferred; strong documentation skills; EBPs.
• Proficiency with electronic health records and MS Office.

Responsibilities

• Maintain an active outpatient therapy caseload.
• Complete Diagnostic Screening Assessments.
• Conduct initial and annual Health Assessments.
• Complete initial and annual Bio-Psychosocial Assessments.
• Develop treatment plans and reviews.
• Prepare continuing care and termination summaries.
• Provide individual, family, and/or group therapy.
• Participate in staff meetings and case conferences.
• Maintain accurate electronic clinical documentation.
• Perform other duties as assigned.

Salary

• Associate Level License: $55,000–$61,875 (LPA, LPCA, CSW, AMFT)
• Independent Level License: $65,000–$73,125 (LPP, LPCC, LCSW, LMFT)

Position Summary

The Outpatient Therapist is responsible for providing assessment, treatment planning, individual and family therapy, and ongoing behavioral health services to assigned consumers. The successful candidate will maintain an active caseload while ensuring timely, accurate clinical documentation in accordance with agency, state, and licensing standards.

Essential Duties & Responsibilities

• Maintain an active outpatient therapy caseload.
• Complete Diagnostic Screening Assessments.
• Conduct initial and annual Health Assessments.
• Complete initial and annual Bio-Psychosocial Assessments.
• Develop individualized Treatment Plans and Treatment/Service Plan Reviews.
• Prepare Continuing Care and Termination Summaries.
• Provide individual, family, and/or group therapy as appropriate.
• Participate in clinical staff meetings, case conferences, and consultations.
• Maintain accurate and timely electronic clinical documentation.
• Perform all other duties as assigned.

Education & Licensure

• Master's degree in Psychology, Counseling, Social Work, or Nursing required.
• Associate or Independent Kentucky licensure required, or actively working toward licensure.
• Employees must comply with all supervision requirements established by their respective licensing board.
• Psychiatric Mental Health Accreditation through the American Nurses Credentialing Center (ANCC) is preferred but not required.

Preferred Qualifications

• Experience working in a behavioral health setting.
• Knowledge of evidence-based behavioral health practices.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Proficiency with electronic health records and Microsoft Office applications.
