Job summary

College Medical Center Phoenix provides outpatient behavioral health and recovery-focused services for individuals with substance use and co-occurring mental health disorders. The therapist conducts assessments, develops individualized treatment plans, and delivers evidence-based counseling through individual, family, and group formats.

Responsibilities include crisis planning, care coordination, and timely clinical documentation.

Qualifications

• Must hold active licensure as MFT, LCSW, or Psychologist; or Associate licensure as required by Arizona Board.
• Minimum 2 years of clinical experience with serious mental illness.
• Minimum 2 years of experience with substance use disorders.
• Strong written and verbal English communication.
• Solid understanding of psychiatric diagnoses and medical record documentation.
• Experience and skill in group facilitation.
• Must complete NCI training within 90 days of hire.
• Must possess a valid Arizona Fingerprint Clearance Card.

Responsibilities

• Conduct patient assessments and develop individualized treatment plans.
• Facilitate group therapy and provide supportive individual counseling within scope of practice.
• Maintain timely, accurate clinical documentation including progress notes and discharge summaries.
• Monitor patient progress, attendance, and participation in treatment.
• Collaborate with physicians, families, case managers, and other providers to coordinate care and discharge planning.
• Respond to crisis situations and implement behavioral interventions as needed.
• Follow program policies, billing procedures, and mandatory reporting requirements.
• Participate in treatment team meetings and complete other duties as assigned.
