Job summary

Community Medical Services (CMS) seeks a Substance Abuse Counselor to support patients recovering from opioid and other substance use disorders in an outpatient setting. You’ll conduct individual sessions, facilitate group counseling, and help develop treatment plans while coordinating with other providers.

Qualifications

• Associates degree or higher required.
• Active LCDC, LPC or LCSW license required.
• Behavioral health experience preferred.

Responsibilities

• Maintain and manage a caseload in compliance with regulatory standards and ratios.
• Conduct individual sessions, facilitate group counseling and didactic educational presentations.
• Complete comprehensive behavioral health assessments and individualized treatment plans, ensuring compliance with regulatory requirements and unique client needs.
• Evaluate the effectiveness of treatment through ongoing assessment of the client and supports, modifying the service plan as needed.
• Coordinate care with recovery and community-based providers; send referrals and complete necessary documentation.
• Engage family and natural supports in the client’s treatment per client preference.
• Maintain knowledge of new developments in substance use and licensure requirements.
• Maintain professional relationships with other substance use professionals and associations.
• Maintain a comprehensive client record and serve as primary contact for coordination with other systems.
• Assist in screening and coordinating primary care services for clients.
