Qualifications

• Bilingual English/Spanish a plus.
• Effective written and verbal communication skills.
• SLP-IL License required.
• CCC and IDFPR licensure required.
• CPR Card required.

Responsibilities

• Evaluates patients, conducts diagnostic evaluations, establishes goals and develops individualized treatment plans.
• Provides counseling and education to patients and family regarding abilities and needs for assistance and continued therapeutic intervention.
• Consults with physicians on evaluations, progress and complications with treatment.
• Maintains medical and departmental records according to policies.
• Communicates with other professional staff and the patient’s family to provide continuity of care.
