Job summary

The Social Worker at St. Tammany Health System intervenes with patients who have complex psychosocial needs, assists in determining eligibility for social programs, and coordinates care planning with an interdisciplinary team.

This role provides crisis intervention to patients and families, helps secure community funds, and ensures the patients receive access to appropriate resources while maintaining confidentiality.

Qualifications

• LMSW or higher licensure required; LCSW preferred.
• Three or more years hospital social work or protective services experience preferred.
• Experience in psychosocial and therapeutic counseling preferred.
• Working knowledge of case management, payors and reimbursement.
• Effective written and verbal communication; professional maturity.
• Basic understanding of Medicare, Medicaid, and Managed Care regulations.
• Organized and able to establish priorities and meet deadlines.
• Self-motivated; requires minimal supervision.
• Ability to work under pressure; demonstrates creativity and flexibility.
• Maintains strict confidentiality of all information and documents.
• Familiarity with community resources.

Responsibilities

• Intervene with patients who have complex psychosocial needs and assist with eligibility determinations for social programs and funding sources.
• Coordinate plan of care for high-risk patient populations with the interdisciplinary team.
• Provide crisis intervention to patients and families as required.
• Refer patients to community funds and agencies.
• Maintain confidentiality of all patient information and documents.
